Full payment is due at the time of service for patients with no Dental Insurance.  Patients with participating insurance, the patient portion is due at this time.

Patients with  HDS, CIGNA, CONCORDIA, DELTA: We are currently a participating provider with these companies and will prepare and submit claim forms for you.

Other Dental Insurance Companies:  We request full payment at the time services are rendered.  As a courtesy service, we will complete the insurance forms and submit them promptly to your insurance company.  Please bring a copy of your insurance form.

As a reminder, most insurance companies DO NOT pay 100% of all dental fees.  Dental insurance is an aid used to pay for part of your obligation.  You will be held responsible for any excess amounts due.
